What is Endovascular Aortic Repair (EVAR)?

Aortic Debranching- a complex Vascular Surgery Procedure

We performed a total aortic debranching of the thoraco-abdominal aorta and then performed an EVAR . In this case, the complete branches to the abdominal organs were bypassed from bilateral iliac arteries. The arteries bypassed were the Coeliac, superior mesenteric, left hepatic, abberant right hepatic artery and both renal arteries. This is termed as  Aortic debranching operation.The patient is doing well

AV Fistula:Dialysis Access for Chronic Kidney Disease Patients

For patients with chronic renal failure, achieving proper vascular access for hemodialysis is crucial. Indwelling intravascular catheters increase the risk of infections and vein thrombosis. The preferred alternative is the Arteriovenous (AV) Fistula, which allows for safer and more effective dialysis.

What is an AV Fistula?

An AV fistula is a direct connection between an artery and a vein, usually in the arm, allowing for high blood flow rates needed for dialysis. This procedure is preferred because it lasts longer and has fewer complications compared to catheters.

Renal transplant is a common operation however it merits the intervention of a vascular surgeon in certain cases especially  if there are multiple arteries supplying the donor kidney. After harvesting the kidney, and giving cold perfusion to wash out all the blood, we have to plan to combine the vessels. The following options have been tried by Dr Avinash Dal very successfully
 
1. In case there are 2 vessels of nearly equal size, one can implant them into the common iliac artery with separate arteriotomies. If, however the vessel are 3mm or less, it is better to perform a “double barreling” operation which means to create a common larger orifice  by joining both the vessels together. Essentially an end on Y-graft. This can be implanted into the external or internal iliac artery
 
2. In case the vessels are of unequal size, then it’s better to anastomose the smaller vessel into the side of the bigger vessel with an end to side anastomosis and then join the larger artery to the external or internal artery
 
3. In case there are three branches which are supplying the kidney, one can do a Y graft or double barreling with 2 of them and with the larger single vessel use as separate anastomosis. For this the internal iliac artery of the donor has to be harvested with its distal branches. Thus you will get one side branch for one anastomosis and one end branch for the other. The internal iliac artery when harvested more distally then usual looks “J” shaped so it has been called the J graft. This has been demonstrated in the picture 
 

Common AV Fistula Sites:

  • Radiocephalic fistula: Wrist region; between the radial artery and the cephalic vein
  • Brachiocephalic fistula: Elbow region : between the brachial artery and the cephalic vein at the elbow. 
  • Basilic vein transposition: Upper arm: in this operation, the basilic vein, which is on the medial side of the arm and draining deep into the accompanying veins of the brachial artery (venae commitantes) . In this operation the veins is brought anteriorly just under the skin and anastomosed at the elbow level to the brachial artery. This requires regional block for the procedure as it is more extensive.
